I am sure you have already figured this out but your power supply has gone bad. it has happened on 6 of my monitors already.

Do you know what version monitor this is? I thought Accuvision came in 36 and 42...never heard of a 40. Power supply is no doubt the issue though. QAMF sells power supply kits for certain version monitors. If you can find the model number on the back...that might point you in the right direction.

Most boards only have 90 days

Many monitor chassis used in different brands are from a common manufacturer and share common modules. The power supplies in flat panel monitors seem to be the primary failure, so maybe there are other options for providing replacements.

I have been told if the item is new...it is a year. If it is rebuilt...90 days. Does that sound right or am I wrong? Curious to know.

If you are good at soldering electronics you can repair the power supply yourself for around $20 in capacitors. Most likely you have two or three that are bulging.
